Job summary

Firefly Children & Family Alliance seeks a full-time Home-based Case Manager in Indianapolis, Indiana. You will serve Marion county, visiting families in their homes and in the community to help them reach their goals and access resources.

You will provide education, transportation, and court involvement when needed, respond to crises, and collaborate with schools and agencies to support youth and families. A Bachelor's degree in social work or related field is required.

Qualifications

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years of age.
• A valid driver's license and insurance with no more than 6 points on driving record.
• Travel between appointments to service clients within their community.
• Knowledgeable regarding social service systems within the state.
• Knowledge of and adherence to current confidentiality practices.
• Appreciation for and ability to respond to cultural differences.
• Develop relevant networks of contacts.

Responsibilities

• Provide family progress monitoring and coaching in the home and community.
• Visit youth/family on a routine and pop-in schedule.
• Assist youth/families in achieving their goals and provide education and support.
• Provide transportation to youth/families and attend court with clients.
• Communicate with schools and institutions related to youth/family matters.
• Respond to crisis calls and manage those situations.
• Create and update plans and goals for youth/family.
• Develop relationships with community agencies and other social welfare organizations.
