Job summary

Firefly Children & Family Alliance is seeking a Home-based Case Manager in Terre Haute, Indiana. This field-based role supports families in their homes, coordinates with schools and social services, and may include evening hours.

You will visit families, attend court, and provide transportation as needed to help clients reach their goals. Ideal candidates hold a Bachelor's degree in social work or related field, are at least 21, and have a valid driver's license with minimal driving points.

Qualifications

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years of age.
• A valid driver's license and insurance with no more than 6 points on driving record.
• Willingness to travel between appointments within the community.
• Knowledge of social service systems within Indiana and confidentiality practices.

Responsibilities

• Provide home-based services to families to reach goals.
• Attend court with clients and render recommendations.
• Provide transportation to youth/families and coordinate with schools and agencies.
