Firefly Children and Family Alliance in Indianapolis, IN seeks a Case Manager to advocate for at-risk children and families, coordinating with internal teams and community agencies to provide shelter care, needs assessments, prevention, intervention, and progress monitoring.

A Bachelor's degree in social work, counseling, psychology, or related field is required; travel between appointments is expected; a valid driver's license and insurance are required; strong communication and confidentiality

• Ability to travel between appointments to support communities.
• A valid driver's license and insurance.
• Knowledge of social service systems within the state.
• Clear, effective communication and adherence to confidentiality.

Responsabilités

• Client Progress - Monitoring and Coaching (50%): Coach collaboratively and support families/youth to achieve their goals.
• Service Plans / Goal Development (20%): Develop and update case plans and monitor placement progress.
• Intake and Needs Assessment (15%): Complete initial assessments and determine crisis situations.
• Reporting (15%): Maintain case files and prepare required written reports.
