Firefly Children and Family Alliance in Terre Haute, IN is hiring a full-time Home-based Case Manager to deliver in-home services, education, and transportation to families in the community. The role is field-based with occasional evening hours and weekend availability.

Requirements include a Bachelor’s degree in social work or a related field, a valid driver’s license, and knowledge of state social service systems; travel between appointments is expected to support client goals.

Qualifications

• Knowledge of social service systems within the state.
• Confidentiality practices and data handling.
• Ability to develop networks of community contacts.

Responsibilities

• Monitor and coach family progress.
• Visit families in their homes on a routine basis.
• Provide education and transportation to clients.
• Attend court with clients and render recommendations.
• Prepare written reports for the appropriate agencies.
