Job summary

Firefly Children & Family Alliance in Indiana is seeking a part-time Home Based Case Manager to serve families in Anderson and surrounding counties. You will perform home visits, provide education and support, and coordinate transportation to help clients reach goals while maintaining confidentiality.

You will attend court with clients, respond to crises, and work with schools and local agencies to stabilize family functioning. Weekend hours are required in this role.

Qualifications

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years of age.
• Valid driver's license with <=6 points on driving record.

Responsibilities

• Family progress coaching with home visits and goals.
• Intake and needs assessment including crisis evaluation.
• Maintain paperwork and write reports for agencies as required.
• Attend court with clients and coordinate with schools and agencies.
• Provide transportation and education/support to families.
• Respond to crisis calls and stabilize family functioning.
