Job summary

Firefly Children & Family Alliance in Indianapolis, IN seeks a full-time Home-based Case Manager to support Marion County families in their homes and communities. You will carry out service plans, provide transportation, attend court, and help families stabilize and reach goals.

The role emphasizes in-home coaching, intake and needs assessment, collaboration with schools and agencies, and maintaining timely reports and confidential records in line with agency standards.

Qualifications

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years of age.
• A valid driver's license and insurance with no more than 6 points on driving record.
• Travel between appointments to service clients within their community.
• Knowledgeable regarding social service systems within the state.
• Knowledge of and adherence to current confidentiality practices.
• Appreciation for and ability to respond to cultural differences.
• Develop relevant networks of contacts.

Responsibilities

• Provide education and support to youth/families and transportation.
• Attend court with clients and render recommendations; communicate with schools and institutions.
• Respond to crisis calls and manage those situations.
• Maintain case and agency paperwork according to standards; prepare reports within identified timeframes.
