Ennoble Care seeks an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Cumberland County, NJ. You will apply medical techniques under physician supervision and care for patients with chronic diseases.

Responsibilities include exams, care planning, and coordinating with the in-house clinical team. 1–2 years of geriatric primary care is preferred; strong independence and communication skills are essential.

Qualifikationen

• Board Certified, with a valid NP or PA certification in New Jersey.
• 1–2 years of primary care experience with geriatric patients (hospice experience preferred).
• Ability to work independently and communicate effectively with patients and staff.

Aufgaben

• Provide high quality primary and palliative care to patients in Cumberland County, NJ.
• Perform physical exams, update histories, and create care plans.
• Communicate with office staff and clinical operations across internal platforms.
• Manage a panel of patients with a patient-centered approach.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications and medically necessary interventions.
