Job summary

Ennoble Care in Sandusky, OH seeks an experienced Advanced Provider (NP/PA) to deliver in-home primary and palliative care to homebound patients. You will work under a physician, applying medical principles to a diverse patient group with chronic diseases as part of a collaborative team.

Qualifications

• Board certified NP or PA with Ohio license.
• Strong clinical skills to diagnose, assess and treat.
• Friendly, collaborative team player with good people skills.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high-quality primary and palliative care in Sandusky, OH.
• Assess health via exams, histories and care plans.
• Communicate with staff and clinical operations across platforms.
• Develop differential diagnoses and treatment plans.
• Consult with patients, families and caregivers.
• Order diagnostic tests as needed.
• Prescribe medications, supplies, and interventions as medically necessary.
