Job summary

Monogram Health is seeking an Advanced Practice Provider (NP/PA) for in-home care across New Jersey. The role involves 3–4 patient visits per week at homes and 1–2 days of telehealth, with a strong emphasis on reducing hospitalizations and improving quality of life.

Base salary ranges $122,653–$202,947 with bonuses and extensive benefits, mileage reimbursement, and ongoing professional growth support. Travel to patient homes is required.

Qualifications

• Demonstrated initiative and independent, critical thinking and decision-making skills.
• Field-based role requiring travel to patient homes, reliable transportation.
• Knowledge of federal/state regulations (HIPAA, OSHA).
• Prior experience with EHR systems such as Athena, Salesforce, Epic.

Responsibilities

• Provide evidence-based, patient-focused care that reduces hospitalizations and improves quality of life in the patient’s home.
• Perform comprehensive assessments including initial evaluations, wellness exams, follow-ups, urgent visits, and post-discharge visits.
• Educate patients and caregivers on disease management, preventive care, medications, and testing.
• Participate in recurring virtual integrated care team meetings.
• Engage in quality assurance activities and document reviews for compliance.
• May practice in SNFs, ALFs, hospitals, and other settings as applicable.
• Perform point-of-care testing, prescribe medications, order tests, and coordinate with Monogram physicians.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
