Job summary

Monogram Health seeks Advanced Practice Providers (NP/PA) to deliver in-home, evidence-based care to patients with multiple chronic conditions. You will work with a multidisciplinary team, providing care at patients’ homes and via telehealth to improve outcomes.

The role includes 3–4 home visits per week, generous compensation commensurate with experience, mileage reimbursement, and opportunities for CEU/CME reimbursement, with paid time off and holidays.

Qualifications

• Active and unrestricted NP or PA license
• Board certification required
• Minimum of two years direct patient care experience
• Field-based role requiring travel to patient homes
• Experience with EHR systems such as Athena, Salesforce, or Epic

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home to reduce avoidable hospitalizations
• Perform comprehensive assessments and wellness exams
• Educate patients and caregivers on disease management and medications
• Participate in virtual integrated care team meetings
• Coordinate care with PCPs, specialists, and ancillary services
• Complete documentation accurately and timely
