Job summary

Monogram Health is seeking compassionate Advanced Practice Providers (NPs or PAs) to deliver high-quality in-home care to patients with multiple chronic conditions in Vineland, NJ. The role works within an integrated care team and includes home visits 3–4 days a week plus telehealth, with no on-call or overnight requirements.

You will perform comprehensive assessments, educate patients, coordinate with PCPs and specialists, and participate in virtual care meetings.

Qualifications

• Active and unrestricted RN and NP license, or Physician Assistant license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate, or eligibility to obtain.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home to reduce hospitalizations and improve quality of life.
• Perform comprehensive assessments including initial evaluations, wellness exams, follow-ups, and post-discharge visits.
• Educate patients and caregivers on disease management, preventive care, medications, and testing.
