Job summary

Monogram Health is seeking Advanced Practice Providers (NPs or PAs) to deliver in-home care to patients with multiple chronic conditions. You will join an integrated multidisciplinary team and provide continuity of care in patients' homes and via telehealth.

Base salary ranges from $122,000 to $158,600, with bonuses and comprehensive benefits. Travel to patient homes is part of the role, with mileage reimbursement and ongoing CME support.

Qualifications

• Active and unrestricted RN and NP or PA license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate or eligible to obtain.
• Minimum of two years of direct patient care experience.
• Field-based role requiring travel to patient homes and reliable transportation.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home.
• Perform comprehensive assessments and wellness visits.
• Educate patients and caregivers on disease management and medications.
• Participate in virtual integrated care team meetings.
• Coordinate care with PCPs, specialists, and ancillary services.
