Job summary
Monogram Health is seeking Advanced Practice Providers to deliver high-quality in-home care to patients with multiple chronic conditions. You will work within an integrated care team, establishing long-term relationships and providing continuity of care in patients' homes and via telehealth.
The role supports a value-based, population health model with opportunities for professional growth. The position includes competitive base salary, incentive bonuses, mileage reimbursement, CEU/CME
Qualifications
- Active and unrestricted RN and NP license, or Physician Assistant license.
- Board certification (NP: ANCC/AANP | PA: NCCPA).
- Current and unrestricted DEA certificate, or eligibility to obtain.
- Minimum of two (2) years of direct patient care experience.
- Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance.
- Knowledge of and compliance with federal/state regulations (HIPAA, OSHA).
- Prior experience with EHR systems (Athena, Salesforce, Epic).
- Background in managed care or population health is preferred.
Responsibilities
- Provide home-based, evidence-based care to reduce hospitalizations and improve quality of life.
- Perform comprehensive assessments and wellness visits.
- Educate patients and caregivers on disease management and medications.
- Participate in virtual integrated care team meetings.
- Coordinate with PCPs, specialists, and ancillary services.
- Complete documentation accurately and timely.
