Qualifications

• Master’s degree in an accredited NP or PA program is required.
• Active NP or PA license and furnishing number is required.
• BLS; NP/PA board certification.
• Experience with older adult care preferred.

Responsibilities

• Conduct in-home assessments for Alignment members.
• Review medications and perform health screenings.
• Educate patients by matching care desires with best care options.
• Identify diagnoses for care management and active medical management.
• Coordinate care with PCPs, specialists, and other providers.
• Lead a clinical team including nurses and care coordinators.
