Job summary

Alignment Health's Advanced Practice Clinician in the Care Anywhere program delivers in-home assessments, medication reviews, and health screenings to members, aiming to prevent hospitalizations and disease progression with timely clinical interventions.

You will partner with the Regional Medical Officer and care team to develop care plans, coordinate with PCPs and specialists, and lead nurses, health coaches and care coordinators to deliver personalized home-based care.

Qualifications

• Master’s degree from an accredited NP or PA program.
• Active NP or PA license with furnishing number and board certification.
• Experience in home health or geriatric care is preferred.

Responsibilities

• Conduct in-home assessments and health screenings.
• Review medications and provide patient education to align care with needs.
• Coordinate care with PCPs, specialists and other providers.
• Lead clinical team including nurses, health coaches and care coordinators.
