Job summary

Jewish Family and Children's Services seeks a Care Manager for its Holocaust Survivor Programs. You will perform in-home and ongoing assessments, develop individualized care plans, coordinate services, and ensure compliance with funders and agency requirements.

Qualifications

• Bachelor’s degree in Social Work, Gerontology, Human Services, Public Health, or related field is required.
• Licensed Clinical Social Worker (LCSW) or higher licensure preferred.
• Valid California Driver’s License and access to reliable transportation; ability to travel to all regions/program sites.

Responsibilities

• Deliver ongoing supportive services to clients and caregivers via phone, video, email, and in-person visits including in-home visits at least every six months.
• Serve as the primary contact for assigned clients and establish communication standards.
• Conduct in-home screenings and comprehensive assessments to evaluate needs and support independence.
• Complete eligibility determinations and required reassessments annually or as needed.
• Develop and monitor goal-oriented, person-centered care plans with clients/families.
• Coordinate referrals, or modifications as needed; advocate for client needs.
• Maintain documentation of all contacts and services to ensure compliance.
