Job summary

Jewish Family and Children’s Services is seeking a Care Manager for Holocaust Survivor Programs to provide comprehensive, trauma-informed support and care coordination for survivors. You will conduct assessments, develop care plans, and coordinate services with families, staff, and community partners while ensuring regulatory and funder compliance.

The role emphasizes client safety, independence, and life quality, with emphasis on documentation and resource stewardship.

Qualifications

• Bachelor’s degree in Social Work, Gerontology, Human Services, Public Health, or related field is required.
• Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist or higher social services licensure preferred.
• Intermediate proficiency in Microsoft Office Suite; experience with EHR, client databases, or CRMs.

Responsibilities

• Deliver ongoing supportive services to clients, families, and caregivers via phone, video, email, and in-person visits, including in-home visits at least every six months.
• Serve as the primary contact for assigned clients; establish communication standards and service quality.
• Conduct in-home screenings and comprehensive assessments to evaluate client needs and maintain safe, independent living where possible.
• Complete eligibility determinations and reassessments for clients annually or as needed.
• Develop, monitor and update care plans with clients and families, coordinating with agency colleagues.
