Job summary

Jewish Family Service of San Diego is seeking a compassionate Care Manager to support Holocaust survivors through person-centered, trauma-informed care management, including intake, assessments, care coordination, and resource navigation in home and community settings.

You will document services for grants and outcomes, maintain databases, and collaborate with the SOS team under the SOS Program Manager. This role emphasizes independence, cultural sensitivity, and ongoing client support.

Qualifications

  • BSW or Bachelor’s degree in Gerontology or human services; MSW preferred.
  • Minimum of two years in care management/resource coordination.
  • Trauma-informed approach for Holocaust survivors.
  • Experience with elderly clients, PTSD, and Jewish culture knowledge.
  • Valid driver’s license and dependable vehicle.

Responsibilities

  • Assess each client using social work skills and complete tracking tools.
  • Determine best fit for client within program eligibility.
  • Use assessment tools and obtain back-up documents per grant requirements.
  • Provide ongoing care management including crisis intervention and advocacy.
  • Attend quarterly Advisory Committee meetings and report progress.
  • Plan and coordinate social events within budget and grant guidelines.
  • Document and monitor all submissions for financial assistance.
