Job summary

Jewish Family and Children's Services of San Francisco, the Peninsula, Marin and Sonoma Counties is seeking a Home Care Supervisor to manage Alzheimer’s survivor services and coordinate client initiation with the Holocaust Survivor Programs team.

You will ensure ADL care plan implementation, staffing alignment, and continuous training compliance, while supporting clients, families, and home care aides with professional, compassionate supervision.

Qualifications

• Russian language proficiency is required.
• Minimum of three years of professional experience in Home Care or Social Services setting is required.
• Strong commitment to providing responsive services to the community and to establishing excellent rapport with clients, staff and community workers.

Responsibilities

• Collaborates with the Care Manager to implement the client care plan through home care visits as directed.
• Develops an individualized care plan at the client’s home in partnership with the client and family.
• Conducts safety assessments during initial consultations with clients, aides, and family members.
• Communicates changes in client condition promptly to the Care Manager or Director to update the care plan.
• Provides in-home supervision and assists in evaluating and supervising caregivers.
• Offers backup coverage for staffing coordination and participates in after-hours on-call rotation.
