Job summary

Jewish Family Service of San Diego is seeking a compassionate Care Manager to support Holocaust survivors in the SOS San Diego program. The role emphasizes trauma-informed care, client intake, assessments, care coordination, and ongoing support in home and community settings.

The position focuses on documentation, grant reporting, and program outcomes using care management databases, with opportunities to connect clients to health, financial, and community resources. Onsite in San Diego.

Qualifications

  • Minimum two years of experience in care management/resource coordination.
  • Experience with trauma-informed care for Holocaust survivors is a plus.
  • Knowledge of benefits for seniors and local resources.

Responsibilities

  • Assess each client using social work skills and complete tracking tools in database.
  • Determine best fit for client within program eligibility requirements.
  • Coordinate home care and financial assistance as needed within grant guidelines.
  • Document and monitor all submissions for financial assistance and program outcomes.
  • Attend advisory and program meetings and provide ongoing care management including crisis intervention.

BSW or Bachelors in Gerontology or human services
