Qualifications

Responsibilities

• Perform psychosocial/physiologic assessments and integrate data.
• Formulate and implement a care plan with patient education and discharge planning.
• Collaborate with patients and families to identify outcomes and priorities.
• Document all patient care activities per standards and assist the team with documentation.
