Responsibilities

• Perform psychosocial/physiologic assessments integrating data.
• Develop and revise care plans based on patient trends and education.
• Coordinate discharge planning and patient education.
• Document all patient care activities per standards.
• Collaborate with the care team to maximize patient outcomes.
• Participate in quality improvement and safety initiatives.
