Responsibilities include assessments, care planning, discharge planning, and collaboration with the healthcare team to ensure quality outcomes. Strong documentation and advocacy for patients are essential.

Qualifications

• Graduate of accredited school of nursing.
• Current Louisiana State license as RN and BLS/ACLS & PALS certifications.
• CPI and NIH Stroke Scale certification within first year of hire.

Responsibilities

• Performs psychosocial/physiologic assessments.
• Formulates a plan of care based on patient trends and education.
• Discharge planning and patient/family education.
• Advocates for patient rights and resolves conflicts.
• Documents care activities per standards and supports team documentation.
