Job summary

LifeWorks Wellness Center is seeking a doctorally-prepared Nurse Practitioner to join our clinical team in a dedicated follow-up and maintenance care role. You will conduct chemo testing follow-ups, lab reviews, rechecks, and annual maintenance visits to ensure timely, high-quality follow-up care as patient volume grows.

You will document interactions in the EMR and collaborate closely with lead physicians.

Qualifications

Active Florida APRN license, board certification, and BLS certification required.
Doctoral degree (PhD or DNP) required.
Strong attention to detail and excellent organizational skills.
Superior interpersonal and communication skills with established patients.
Ability to align with lead physicians during and after training period.

Responsibilities

Manage follow-up, recheck, lab reviews, and annual/maintenance visits for established patients across multiple physicians.
Conduct chemo testing follow-ups, 6-week lab reviews, and standard follow-up visits per protocol.
Track patient progress, identify setbacks, and adjust care with the practitioner team.
Document all patient interactions accurately in the EMR and maintain continuity of care.
Respond promptly to emails and questions from RNs and patients, supporting care protocols.
