Qualifications

• Masters Degree in Nursing or Physician Assistant Studies.
• Valid NP or PA license in the operating state with current board certification.
• Strong leadership and problem-solving abilities.
• Excellent written and oral communication skills.
• Punctuality and organizational skills; team player.

Responsibilities

• Perform comprehensive patient assessments and manage chronic conditions.
• Conduct routine rounds and adjust treatment plans.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and families on disease prevention and wellness.
• Maintain EMR documentation and ensure regulatory compliance.
• Support quality improvement and reduce hospital readmissions.
