Job summary

• Clinical Practice and Care Management.
• Performs psychosocial/physiologic assessments that integrates changing data.
• Formulates a plan of care based on trends of similar patients, including patient education and discharge planning.
• Participates in planning of routine transitional health care needs, including end of life options.
• Responds to changes in patient's needs with holistic care using independent clinical judgment.
• Adapts teaching to individual patients and families, integrating education during care delivery.
• Assists the healthcare team with documentation and maintains documentation standards.
• Communicates and collaborates with patients, families and health care team to maximize outcomes.
• Delegates care activities and participates in staffing decisions to achieve productivity.
• Maintains knowledge of unit initiatives and participates in committees/work groups.
• Participates in quality improvement initiatives and utilizes standards and pathways for care delivery.
• Incorporates evidence-based practice and research findings into nursing practice.
• Acts as a clinical resource and mentor to health care team members and supports lifelong learning.
