Job summary

Catholic Charities Milwaukee is seeking a dedicated professional to conduct in-home assessments and understand clients’ needs. You will collaborate with Home Coaches and Therapists to formulate personalized care plans, and assist clients with de-cluttering, organizing, and accessing essential services as outlined in their care plan.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
• At least six months of relevant experience.

Responsibilities

• Conduct client inquiries and in-home assessments to understand clients’ physical, social, and financial needs.
• Collaborate with Home Coaches and Therapists to develop individualized care plans.
• Assist clients one on one in their home with environmental de-cluttering, organization, and other support as outlined in the care plan.
• Help clients access essential services (e.g., Medicare, food assistance, family care, disability programs).
• Monitor client progress, evaluate treatment outcomes, advocate for additional resources, and provide personal support during crises.
