Job summary

Catholic Charities of the Archdiocese of Milwaukee is hiring a Hoarding Case Manager to guide clients through an in-depth Hoarding Intervention and Treatment Program. You’ll assess needs, develop individualized care plans with Home Coaches and Therapists, and help clients access essential services to achieve safe, stable housing.

The role emphasizes hands-on in-home support, collaboration with a multidisciplinary team, and compassionate advocacy for people facing hoarding-related challenges.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field required.
• At least six months of experience in a social service or health care setting required; 2+ years preferred.

Responsibilities

• Assess client needs through in-home assessments to understand physical, social, and financial needs.
• Build individualized care plans with Home Coaches and Therapists.
• Provide hands-on support in clients' homes, assisting with decluttering and organization.
• Connect clients to essential resources such as Medicare, food assistance, Family Care, and disability programs.
• Monitor progress, advocate for additional resources, and provide support during crises.
