Job summary

Catholic Charities of the Archdiocese of Milwaukee is seeking a Hoarding Case Manager to guide clients through our Hoarding Intervention and Treatment Program. You will assess needs, develop care plans with Home Coaches and Therapists, connect clients to Medicare, food assistance, and disability programs, and provide in-home support to declutter and organize belongings.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• At least six months of experience in a social service or health care setting; 2+ years preferred.
• Experience with older adults, disabilities, or people with cognitive, physical, or mental health challenges.
• Valid driver's license, reliable transportation, and vehicle liability insurance required.

Responsibilities

• Assess Client Needs: Respond to client inquiries and conduct in-home assessments to understand physical, social, and financial needs.
• Build Individualized Care Plans: Collaborate with Home Coaches and Therapists to develop a care plan.
• Provide Hands-On Support: Work one-on-one with clients in their homes, helping with decluttering and organizing.
• Connect Clients to Resources: Help clients access Medicare, food assistance, Family Care, and disability programs.
• Monitor Progress and Advocate: Evaluate outcomes, advocate for resources, and provide personal support during crises.
