Qualifications

Responsibilities

• Conduct client inquiries and in-home assessments to understand clients' needs.
• Collaborate with Home Coaches and Therapists to develop care plans.
• Assist clients with environmental de-cluttering and organization.
• Monitor client progress and advocate for additional resources.
