Job summary

Neighborhood Health seeks a dedicated professional to support HIV and hepatitis screening programs, linking patients to care and prevention services. You will coordinate outreach, arrange testing and follow-up, and help address barriers to care while ensuring accurate documentation and data quality.

Requires frontline experience in patient navigation or care coordination, with strong communication skills and cultural competence.

Qualifications

• Bachelor’s degree in public health, social work, health sciences, or related field and/or 1–3 years of relevant experience in patient navigation, care coordination, case management, or community health services.
• Experience with HIV, viral hepatitis, sexual health, or prevention services preferred.
• Excellent interpersonal and cross-cultural communication skills and ability to work with diverse populations.

Responsibilities

• Facilitate routine HIV, HBV, and HCV screening and timely linkage to care.
• Coordinate outreach, appointments, and lab testing using eClinicalWorks and Azara.
• Monitor referral tracking lists and ensure timely provider follow-up.
• Link patients to care and to Ryan White services and specialty providers.
• Engage patients out of care to re-engage them with scheduling and care teams.
• Provide HIV prevention education and PrEP information.
