Job summary

La Clinica de La Raza seeks a Clinical Health Navigator to join HIV services. Candidates should have a health‑related degree and experience in HIV care, with strong communication skills. BLS and HIV certifications are preferred.

Qualifications

• High School diploma or GED required.
• Bachelor’s degree in a health-related field preferred or highly desired.
• Associates degree accepted with relevant experience.
• Experience in HIV-related work or healthcare settings is preferred.

Responsibilities

• Conduct HIV testing and outreach to identify clients.
• Navigate newly diagnosed or out‑of‑care individuals into HIV medical care.
• Coordinate appointments, labs, and referrals with the medical team.
• Maintain HIPAA compliance and accurate medical documentation.
• Provide education on HIV, PrEP/PEP, and retention in care.
• Assist with enrollment, insurance access, and pharmacy needs.
• Support linkage to long‑term preventive care and case management.
