Job summary

Hyacinth AIDS Foundation is seeking a dedicated outreach specialist to implement and evaluate outreach services for HIV care. Under supervision, you will coordinate with clinics, reconnect clients to medical services, and maintain confidential case records. You will engage with diverse communities and represent the agency at events.

The role emphasizes collaboration, HIPAA compliance, and building partnerships to improve access to care and support for clients living with HIV.

Qualifications

• Demonstrated ability to work with people from diverse backgrounds and interests.
• Proficiency in written and verbal communications.

Responsibilities

• Collaborate with staff in local clinics to outreach to out of care individuals and reconnect them to HIV medical services and other supportive services.
• Refer and link undiagnosed clients to rapid HIV testing services.
• Maintain client case records and ensure confidentiality of all records according to HIPAA.
• Represent the agency at community events, meetings, and with external partners.
