Qualifications

  • Minimum of 5 years of relevant experience required.
  • Strong knowledge of viral hepatitis, HIV, addiction services, and care coordination.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work independently, build community partnerships, and provide compassionate, nonjudgmental support.

Responsibilities

  • Provide case management for patients with viral hepatitis from linkage to care through treatment completion.
  • Coordinate referrals and support access to medical, social, and community resources.
  • Conduct outreach and linkage-to-care services at community screening events and partner sites.
  • Support patients needing both hepatitis and HIV care coordination.
  • Develop and maintain patient tracking systems and referral workflows.
  • Assist with staff training, resource development, and program implementation.
  • Reconcile patient support fund expenses, including transportation and medication assistance.
  • Collaborate with hospitals, community agencies, and addiction treatment centers on education and outreach.
