Job summary

Penn State Hershey Medical Center seeks a dedicated HIV/STI Prevention Navigator to coordinate prevention, testing, treatment, and supportive services within Infectious Disease. The role focuses on reducing barriers to care through transportation, insurance assistance, and appointment scheduling.

The Navigator will engage with community partners to improve health equity, provide PrEP/PEP education, and connect patients to resources, contributing to long-term health outcomes.

Qualifications

• Bachelor’s degree in a health-related field.
• Minimum of two years in healthcare, public health, case management, patient navigation, social services or outreach.

Responsibilities

• Serve as central contact for HIV/STI prevention, testing, and care services.
• Educate on HIV prevention strategies, including PrEP and PEP.
• Coordinate testing, linkage to care, and access to community resources.
• Address transportation, insurance, medication assistance, appointments, and social determinants of health.
