Job summary

Penn State Health is seeking a Sexual Health & HIV Prevention Navigator at Milton Hershey Medical Center in Hershey, PA. The role serves as a central contact for HIV/STI prevention, testing, treatment, and care, guiding patients through PrEP/PEP education, testing, and linkage to services.

The Navigator addresses barriers to care, including transportation, insurance, and medication assistance, and builds partnerships with community organizations to improve engagement and health outcomes.

Qualifications

• Bachelor’s Degree in Social Work, Nursing, Public Health, Health Education, Human Services, or a related field.
• Minimum of two (2) years of experience in healthcare, public health, case management, patient navigation, social services, or community outreach.
• Preferred: CHTC credential or ability to obtain certification within designated timeframe.
• Experience with PrEP navigation, linkage-to-care programs, Ryan White services, medication assistance programs, or healthcare access initiatives.
• Knowledge of HIV/STI surveillance, outreach strategies, and risk-reduction interventions.
• Experience conducting community education, health promotion, or patient engagement activities.
• Familiarity with underserved, high-risk, or vulnerable populations affected by HIV and other sexually transmitted infections.
• Bilingual or multilingual language skills.
• Experience using electronic health records (EHR/EMR), case management software, and data tracking systems.
• Demonstrated ability to collaborate with healthcare providers, community organizations, and public health agencies to improve patient outcomes and access to care.

Responsibilities

• Serve as a central contact for HIV and STI prevention, testing, treatment and care services.
• Provide education on prevention strategies including PrEP and PEP.
• Coordinate testing and treatment services and facilitate linkage to medical and behavioral healthcare.
• Connect patients with community resources and address barriers to care (transportation, insurance, medication assistance, scheduling).
• Conduct community outreach and build partnerships with local organizations, educational institutions, and public health agencies.
• Promote health equity and reduce stigma through engagement with underserved populations.
