Job summary

Community Health Action of Staten Island (CHASI) seeks a Navigation specialist in the Prevention Department to provide case management and referrals for priority populations focused on HIV prevention and connecting clients to PrEP, PEP and care.

Responsibilities include intakes, screenings, developing integrated service plans, scheduling appointments, maintaining accurate notes, and collaborating with partner providers.

Qualifications

• 2+ years experience in case management or health education with at-risk populations.
• Bilingual English/Spanish preferred.
• Strong written and verbal communication skills.

Responsibilities

• Conduct intakes, screenings, and assessments to identify client needs.
• Educate and counsel on prevention strategies and materials.
• Develop and coordinate integrated service plans with clients.
• Assist with appointment scheduling, transportation, and follow-ups.
• Maintain detailed and timely case notes and records.
• Collaborate with partner providers for referrals and services.
• Support outreach events and community engagement activities.
• Occasionally work nights and weekends as needed.
