Job summary

Apicha Community Health Center is seeking a full-time Patient Navigator in Manhattan. The role focuses on field-based case management for patients enrolled in HIV care coordination, serving as the main contact to help individuals engage in prevention and treatment services.

The navigator works with care team members to remove barriers to care, identify resources, and support patients in navigating health care systems to achieve optimal health outcomes.

Qualifications

• Associate’s degree in human services or social work with 12+ months experience in patient navigation.
• May consider certification as a peer worker by NYS AIDS Institute Peer Certification, or CUCS case management certification, or at least 90 hours of documented training equivalent to those required by the NYS Peer Worker.
• Strong socio-cultural identification with the target population or experience with the target population.
• Fluency in one or more Asian language preferred; excellent written and verbal English communication.
• Ability to communicate well with medical providers and support staff; capable of handling multiple tasks in a stressful environment.

Responsibilities

• Provide direct services to people living with HIV/AIDS under supervision of Care Coordinator.
• Execute tasks to support medical and social service plans; accompany patients to appointments when required and provide health education.
• Observe and report ARV self-administration and ARV-related side-effects; assist in home or community settings as needed.
• Maintain client records and documentation per APICHA protocols.
• Work as part of CQI activities and represent the agency at various venues to promote services.
• Other activities as assigned.
