Job summary

Hyacinth AIDS Foundation in New Jersey is seeking a Community Health Worker in West New York to support people living with HIV, connect them to medical care, and coordinate related services.

Qualifications

• Ability to work with people from diverse backgrounds.
• Strong written and verbal communication skills.
• HIV/AIDS knowledge or experience preferred.

Responsibilities

• Collaborate with local clinics to outreach to out of care individuals and reconnect them to HIV medical services.
• Provide information on living with HIV, prevention, and access to services.
• Escort clients to medical appointments and assist enrollment in care.
• Maintain confidential client records per HIPAA guidelines.
• Represent the agency at community events and coordinate referrals.
