Job summary

Hyacinth Foundation in Union City, NJ seeks a TLC Navigator to counsel clients on HIV infection and testing and to lead outreach activities for at-risk populations. The role focuses on pre/post-test counseling, risk reduction planning, and referral tracking within a supportive, confidential environment.

The ideal candidate will have completed Alere and INSTI trainings, hold a relevant degree with experience in HIV/AIDS, and demonstrate cultural competence and strong communication skills.

Qualifications

• Bachelor’s degree plus one year of HIV/AIDS related experience, or more years of relevant experience in lieu of degree.
• Completion of Alere and INSTI trainings.
• Knowledge of HIV/AIDS prevention in diverse communities.
• Access to a registered car and a valid driver’s license.
• HIV antibody test counselor training or at least one year of HIV test counseling experience.
• Training/experience in a helping field (psychology, social work, counseling).
• Experience with cultural competence for client populations to be served.
• Comfort discussing sex frankly in everyday language.
• Completion of NJDHSS-DHAS Counseling & Testing course.
• Ability to work flexible hours including evenings and weekends.
• Proficiency in written and verbal communications.

Responsibilities

• Conduct pre- and post-test counseling sessions with clients.
• Develop risk reduction plans for clients seeking HIV counseling and testing.
• Refer and track clients in need of internal/external services.
• Conduct HIV testing using universal precautions and follow state testing regulations.
• Complete documentation for testing activities in a timely manner.
• Maintain and update client case records using agency forms.
• Ensure confidentiality of all records.
• Prepare monthly reports per agency policies.
