Job summary

Boston Health Care for the Homeless Program is seeking an Outreach Social Worker/Coordinator for its HIV Primary Care team. This role blends direct social work, behavioral health, outreach coordination, and program administration to engage and retain patients in comprehensive HIV care, regardless of housing status.

• Licensed clinical social work experience preferred in Massachusetts.
• Experience with trauma-informed care and motivational interviewing.
• Familiarity with restorative justice principles is preferred.

Responsibilities

• Lead weekly outreach activities within the HIV team and collaborate with partners.
• Develop therapeutic relationships to promote engagement and continuity in care.
• Screen referrals for ARCH program and manage documentation and reporting.
• Provide supervision and support to ARCH Outreach Case Manager.
• Participate in weekly team meetings and cross-agency collaborations.
