Qualifications

• Bachelor's degree in a human service field and one year case management experience is required.
• HIV knowledge or experience is a plus.
• Must be responsive to the needs of the target population.
• Excellent verbal and written communication; ability to function as part of a team.
• Can handle a flexible work environment; have strong decision-making skills.

Responsibilities

• Conduct intakes and assessments according to program requirements.
• Develop, implement, and monitor individual service plans.
• Meet in secure locations outside the office.
• Complete reports and documentation for billing and regulatory needs.
• Advocate for consumers and monitor quality outcomes.
• Maintain timely documentation for billing of services.
• Notify supervisory staff of service gaps and coordinate needed services.
• Participate in program-related training and related duties.
