Job summary

LGBT Life Center in Norfolk, VA seeks a Medical Case Manager to provide comprehensive, client-centered services to individuals living with HIV/AIDS and the broader LGBTQ+ community. You will facilitate access to medical care, behavioral health services, housing resources, and essential support in a collaborative multidisciplinary team.

Responsibilities include ISP development, care coordination, documentation, and community outreach to promote adherence and housing stability.

Qualifications

• Bachelor's degree preferred; combinations considered in lieu of degree.
• Minimum of two years of experience in case management or related field.
• Experience with individuals living with HIV/AIDS preferred.
• Experience serving vulnerable populations, including LGBTQ+ individuals and those affected by homelessness or mental health issues, highly desirable.

Responsibilities

• Provide comprehensive case management and supportive services to individuals and families.
• Develop, implement, and monitor ISPs tailored to each client's needs.
• Coordinate internal and external services to support medical, behavioral health, housing stability, and program requirements.
• Conduct regular client meetings, including home visits and office appointments, to assess progress.
• Monitor adherence to program guidelines and provide targeted interventions.
• Connect participants to employment opportunities, vocational training, and financial literacy resources.
• Assist clients in identifying strategies to increase income and reduce expenses for housing stability.
