Job summary

St. Luke's University Health Network is seeking a Medical Case Manager to provide comprehensive case management services to persons with HIV/AIDS within the HOPE Program.

The role involves intake, assessment, care coordination, referrals, and ongoing monitoring while maintaining confidentiality and complying with funding requirements. The position requires a Bachelor’s degree in social work, psychology, or sociology and at least one year of HIV/AIDS management experience is preferred.

Qualifications

• Bachelor’s degree in social work, psychology, or sociology or related field.
• Minimum one year HIV/AIDS management experience preferred.
• Knowledge of HIV risk behaviors and behavior change principles preferred.
• Bilingual capability—Spanish language preferred.

Responsibilities

• Provide case management services to persons with HIV/AIDS.
• Intake and screening, assessment, and development of service plans.
• Coordinate referrals, monitor plan effectiveness and compliance, and advocate for clients.
• Maintain accurate client records and documentation per policies and funding agreements.
• Collaborate with physicians, nurses, and other health care team members to support interdisciplinary care.
