Job summary

NYC Health + Hospitals is seeking a grant-focused coordinator to advance HIV prevention and Hepatitis initiatives across its clinical sites. You will coordinate navigators, support clinical leads, and ensure timely data collection and reporting for grant deliverables.

You will collaborate with public health partners to design quality improvement activities, train staff, and coordinate with health departments, with a focus on underserved populations and health equity throughout NYC.

Qualifications

  • Bachelor's degree in a health administration, public health, or related field.
  • Four (4) years of satisfactory, full-time experience related to the project objectives in a health care or hospital setting.
  • Satisfactory equivalent in education, training and/or experience.

Responsibilities

  • Coordinates and oversees grants supporting HIV prevention and Hepatitis programs across NYC H+H clinical sites.
  • Collaborates with clinical and administrative leads to identify, design and implement quality improvement initiatives.
  • Provides trainings and updates to patient-facing teams, including medical providers and support staff.
  • Manages quarterly grant reporting requirements.
  • Works with NYC Department of Health on hepatitis initiatives including Hep Free NYC coalitions.
  • Develops and implements evaluation tools to track progress toward program goals.
  • Completes required data collection, reporting and other grant deliverables.
  • Establishes working groups to address clinical or procedural concerns and participates in practice improvement meetings.
  • Creates feedback mechanisms to sustain the program and communicate with leaders.
  • Strategizes with clinics to improve engagement for individuals with substance use disorders, justice-involved individuals, homelessness, and mental health co-morbidities.
