Job summary

NYC Health + Hospitals is seeking a grant-focused coordinator to advance HIV prevention and Hepatitis initiatives across its clinical sites. You will coordinate navigators, support clinical leads, and ensure timely data collection and reporting for grant deliverables.

You will collaborate with public health partners to design quality improvement activities, train staff, and coordinate with health departments, with a focus on underserved populations and health equity throughout NYC.

Qualifications

• Bachelor's degree in a health administration, public health, or related field.
• Four (4) years of satisfactory, full-time experience related to the project objectives in a health care or hospital setting.
• Satisfactory equivalent in education, training and/or experience.

Responsibilities

Duties & Responsibilities

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

Coordinates and oversees efforts related to five grants supporting high quality, equitable HIV prevention and Hepatitis programs across NYC H+H clinical sites, with a particular focus on populations disproportionately impacted by the epidemics of HIV and Hep C.

Standard work responsibilities include: coordinating and supporting the efforts of facility-based Hepatitis Navigators; collaborating with clinical and administrative leads to identify, design and implement quality improvement initiatives; provide trainings and updates to patient-facing teams, including medical providers and support staff; managing quarterly grant reporting requirements; and collaborating with NYC Department of Health on hepatitis initiatives including participating in Hep Free NYC coalitions.

General Tasks And Responsibilities Will Include

• Provide project management and subject-matter support for grant-funded activities related to HIV pre-exposure prophylaxis (PrEP), and hepatitis screening, care linkage and treatment.
• Work collaboratively with clinical and administrative leaders, mid-level providers and Patient Navigators to advance Hepatitis C navigation with the goal of increasing screening and cure rates.
• Understand clinic-level operational processes (care team staffing models, clinical flow processes, clinic design, scheduling and templates, visit types, front desk processes, referral tracking, missed visit follow up) to support development of effective and efficient initiatives to improve quality and access related to grant programs.
• Maintain understanding of clinical standards (state and federal) related to HIV prevention and Hepatitis.
• Develop and implement evaluation tools to track progress toward meeting program goals.
• Complete all required data collection, reporting and other grant deliverables.
• Establish working groups to address specific clinical or procedural concerns to ensure systems are reflective of the needs of patients and staff, including participating in relevant practice improvement meetings/sessions.
• Create feedback mechanisms to support sustainability of the program and ensure facility/clinic administrative and clinical leaders are kept informed and have their concerns addressed.
• Strategize with clinics to improve engagement for individuals with substance use disorder, people involved in the justice system, individuals experiencing homelessness, and people with mental health co-morbidities.

Minimum Qualifications

• A bachelor's degree from an accredited college or university in a specialization or discipline consistent with project content; and
• Four (4) years of satisfactory, full-time experience related to the objectives of the project in a health care or hospital setting; or
• A satisfactory equivalent of education, training and/or experience.

Department Preferences

• Knowledge and experience related to organizational change in healthcare and/or implementing process improvement projects.

  • At least 10 years of experience in areas that include program implementation, designing and providing trainings, program management, quality improvement, grant management, and/or clinical operations preferred
  • Advanced program management skills, ensuring all deadlines are met and organizing initiatives across a large public health care system; with excellent communication and training skills, supporting collaborations and trainings with various clinical partners; and an innovative and experienced problem solver, understanding the challenges inherent in implementing new clinical processes and overcoming both the small and large obstacles to success.
