Job summary

Henry Ford Health HIV Health Navigator role provides comprehensive support and care coordination for individuals living with HIV or at high risk, helping clients access medical care, treatment, prevention services, and community resources while promoting treatment adherence and engagement in care.

You will collaborate with physicians, nurses, social workers, case managers, pharmacists, and community organizations to remove barriers to care, document in EMR, and support insurance navigation

Qualifications

• Bachelor's degree in Social Work, Public Health, Psychology, Human Services, Nursing, or related field preferred.
• Certification as a Community Health Worker (CHW) or HIV specialist preferred.
• Experience with Ryan White HIV/AIDS Program services; bilingual skills preferred.

Responsibilities

• Assist clients in accessing HIV-related medical care, including primary and specialty care.
• Conduct client needs assessments and develop individualized care plans.
• Educate clients about HIV prevention, treatment, medication adherence, and available support services.
• Coordinate referrals to healthcare providers, mental health services, housing, transportation, and financial resources.
• Monitor client progress and follow up to ensure engagement in care and successful referrals.
• Provide advocacy to reduce barriers to healthcare access.
• Maintain accurate documentation in electronic health records and case management systems.
• Collaborate with physicians, nurses, social workers, case managers, pharmacists, and community organizations.
• Assist with linkage-to-care and retention-in-care initiatives.
