Job summary

Greater Family Health in the Chicagoland area is hiring an experienced Medical Case Manager (HIV/AIDS) to support people living with HIV/AIDS. You will assess eligibility, develop personalized care plans, and coordinate access to services.

You will monitor outcomes, link clients with primary care providers, document services, follow AIDS Foundation SOPs, participate in trainings, and contribute to a team focused on patient-centered care.

Qualifications

• Bachelor's degree in related field required.
• BSW or social work degree preferred for the role.
• BLS certification must be active within 30 days of start date.
• Experience with case management and writing skills is desirable.

Responsibilities

• Complete initial eligibility assessment for clients.
• Develop comprehensive, individualized care plans with short- and long-term goals.
• Coordinate access to health and support services and ensure continuity of care.
• Monitor progress and efficacy of the care plan.
• Reference the client’s primary care provider and coordinate referrals.
• Enter and track services in the client-level database and adhere to SOPs.

If this is you, keep on reading.Your Goal:The Medical Case Manager provides medical case management services for people living with HIV/AIDS.How You'll Help Patients:Compete initial eligibility assessment.Development of comprehensive, individualized care plan jointly with the client/patient that includes short- and long-term goals focused on attaining, maintaining, and achieving positive health outcomes.Facilitate timely and coordinated access to medically appropriate levels of health and support services and continuity of care.Provide continuous client/patient monitoring to assess the efficacy of the individualized care plan.Coordinate the individualized care plan with client’s/patient’s primary care provider.Refer and link client/patient to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Monitor the client’s follow through with these services.Enter and utilize the client/patient level database for the tracking and reporting of all services provided.Follow the Standard Operating Procedures of the AIDS Foundation of Chicago’s Northeastern Illinois HIV/AIDS Case Management Collaborative.Engage in all offered training, consultations and supervisions made available.
