Job summary

Greater Family Health in Elgin, IL is seeking an experienced Medical Case Manager (HIV/AIDS) to provide medical case management services for people living with HIV/AIDS. You will contribute to the community and impact patients' lives.

Responsibilities include eligibility assessments, development of comprehensive, individualized care plans, coordinating care with primary providers, and monitoring outcomes. A Bachelor’s degree and BLS within 30 days are required; bilingual Spanish is preferred.

Qualifications

• Bachelor’s degree in related field is required.
• Active and current Basic Life Support (BLS) certification within 30 days of start date is required.
• Bilingual in Spanish is strongly preferred but not required.
• Minimum 1 year experience in Case Management is highly preferred but not required.
• Excellent writing skills are expected.
• Computer literate and able to utilize various software, word processing and spreadsheets.

Responsibilities

• Conduct initial eligibility assessment.
• Develop comprehensive, individualized care plans with the client.
• Coordinate access to appropriate health and support services.
• Monitor progress and adjust care plans as needed.
• Coordinate care with client’s primary care provider.
• Refer to and link clients to community services; track follow-through.
• Enter data into client level database for reporting.
• Follow established SOPs of AIDS Foundation of Chicago program.

Your Goal:

The Medical Case Manager provides medical case management services for people living with HIV/AIDS.

How You'll Help Patients:

• Compete initial eligibility assessment.
• Development of comprehensive, individualized care plan jointly with the client/patient that includes short- and long-term goals focused on attaining, maintaining, and achieving positive health outcomes.
• Facilitate timely and coordinated access to medically appropriate levels of health and support services and continuity of care.
• Provide continuous client/patient monitoring to assess the efficacy of the individualized care plan.
• Coordinate the individualized care plan with client’s/patient’s primary care provider.
• Refer and link client/patient to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Monitor the client’s follow through with these services.
• Enter and utilize the client/patient level database for the tracking and reporting of all services provided.
• Follow the Standard Operating Procedures of the AIDS Foundation of Chicago’s Northeastern Illinois HIV/AIDS Case Management Collaborative.
• Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.
• Perform other duties as assigned.

Qualifications: What You'll Need:
