Job summary

Mazzoni Center is seeking a Medical Case Manager to assess the needs of individuals living with HIV and connect them to essential medical and psychosocial services. The role includes monitoring, follow-up, and coordination of ongoing care to improve client outcomes.

Applicants should have a Bachelor’s Degree in Social Services and knowledge of HIV/AIDS, with strong communication skills and the ability to work with diverse populations. On-site work in Philadelphia is required.

Qualifications

• Bachelor’s Degree in Social Services (Psychology, Social Work, Nursing, Public Health).
• Knowledge of HIV/AIDS and community resources.
• LGBTQ cultural competency.
• Competency in working with diverse populations.
• Ability to travel around a city location regularly.
• Ability to use a computer for up to 6 hours per day.
• Vaccination requirements per Mazzoni Center policy.

Responsibilities

• Assess the needs of individuals living with HIV and link them to medical and psychosocial services.
• Provide monitoring, follow-up, and coordination of services.
• Maintain accurate written documentation of client contacts (DAP format).
• Protect client information and records in line with HIPAA and confidentiality policy.
• Participate in supervision, trainings, and agency meetings.
• Assist with food bank deliveries and other program duties.
